I don't get the logic in including extra rows for the alternate formes of some Pokémon (ie: Giratina) but not for others (ie: Unown). It's rather inconsistent. Moreover, it's disconcerting to see some Pokémon listed multiple times, as when looking at the table, one expects its different "slots" to belong to different Pokémon.
