Can be used to evolve one of several Pokémon by offering it at the [[Luminous Cave]]{{sup/md|RB}} or [[Luminous Spring]]{{sup/md|TDS}} or by using it in a dungeon{{sup/md|GTI}}. This consumes the Moon Stone.

If thrown, it will deal 1{{sup/md|RB}}{{sup/md|TDS}}/2{{sup/md|GTI}} damage (2 if [[sticky]]<!--confirm for GTI-->). In Red Rescue Team, Blue Rescue Team, Explorers of Time, Explorers of Darkness, and Explorers of Sky, using this item in a dungeon will consume it, with no effect.

In ''[[EP006|Clefairy and the Moon Stone]]'', a giant Moon Stone appeared deep within [[Mt. Moon]], being worshipped by a group of {{p|Clefairy}} who lived there. It was soon stolen by {{TRT}}, and ultimately ended up being blown up; the shards from the shattered giant Moon Stone rained down on the Clefairy and caused some of them to evolve into {{p|Clefable}}. In this episode, the characters speculated that the Moon Stone was how various Pokémon—Clefairy in particular—arrived in the Pokémon world. Another giant Moon Stone was later seen in the same location in ''[[AG134|A Real Cleffa-Hanger]]''.

In ''[[BW135|Mystery on a Deserted Island!]]'', the Moon Stone was featured along with all of the other evolutionary stones available as of [[Generation V]].

In [[PS007|the next round]], [[Team Rocket]] searched for one in [[Mt. Moon]]. However, it was {{adv|Red}} who eventually found it. In ''[[PS033|The Winged Legends]]'', Red used his Moon Stone to evolve {{adv|Green}}'s {{p|Clefairy}} into a {{p|Clefable}} in order to fight {{DL|Legendary birds (Adventures)|Thu-Fi-Zer}} during the [[Silph Co.]] showdown with Team Rocket.

In ''[[PS194|Guile from Mawile]]'', {{adv|Ruby}}'s [[Kiki]] evolved into a {{p|Delcatty}} due to being exposed to one of {{Steven}}'s Moon Stones. Another one of Steven's Moon Stones was seen in ''[[PS196|Ring Ring Goes Beldum]]''.

In ''[[PS288|Give It Your Best, Blastoise]]'', during the battle against the {{adv|Deoxys}} Duplicates, {{adv|Green}} evolved her [[Jiggly]] and Nido into {{p|Wigglytuff}} and {{p|Nidoqueen}}, respectively, using two Moon Stones.
