|effect=Once during your turn ''(before your attack)'', you may search your deck for another {{TCG|Deoxys}} and switch it with {{tt|Deoxys|this Pokémon}}. (Any cards attached to {{tt|Deoxys|this Pokémon}}, damage counters, Special Conditions, and effects on it are now on the new Pokémon.) If you do, put {{tt|Deoxys|this Pokémon}} on top of your deck. Shuffle your deck afterward. You can't use more than 1 Form Change Poké-Power each turn.

This card was included in the {{TCG|EX Holon Phantoms}} expansion with artwork by [[Kouki Saitou]], first released in the Japanese {{TCG|Holon Phantom}} expansion. In Japan, this card was later reprinted with different artwork by the same artist as one of the {{TCG|PCG-P Promotional cards}} and was awarded to participants of the regional Spring 2006 Battle Road tournaments that progressed from the preliminary round. The English print was released in {{TCG|POP Series 4}}. A Cosmos Holofoil version of the POP Series 4 print was one of two cards available in the {{TCGMerch|EX|Series|EX Power Keepers Three Pack Blisters}}, released in March 2007.

 

The Form Change Poké-Power references the ability of {{p|Deoxys}} to change into [[List of Pokémon with form differences#Deoxys|multiple formes]]. This card features the Normal Forme of Deoxys.
