While a single Pokémon cannot hold two items at once, therefore it would make it impossible for a single Pokémon to both Mega Evolve and use a Z-Move, there is one exception to that rule. Rayquaza who is not required to have an item held for mega evolution since Rayquaza devoured the Mega Stone. Therefore, this leaves its item slot held open for a Z-Crystal. Mind you, my basis for this fact is based on my knowledge of Mega Stones in the XY games and my knowledge of Z-Crystals in the Sun/Moon games.
