This is a Select button glitch, similar to the Dokokashira door glitch.  To perform the glitch, the player must have at least 13 separate items and a Magikarp in the first slot in his party.  The player must open the Items menu and press Select on the 13th item, then exit the menu by pressing B twice.  Then the player must open the Pokémon menu without selecting it from the main menu (either talk to an NPC that brings it up, such as the Day Care man or the Name Rater, or by getting into a battle and going to switch the Pokémon).  The player must then press A on the Magikarp and it will change into Mew.
However, this glitch causes problems with the Pokédex, which makes it so that the Mew's Pokédex entry cannot be viewed.  Also, the Mew obtained this way is severely glitched.  The name becomes a string of seemingly random characters, which cannot be changed due to the fact that the ID No. and the Original Trainer also end up changed.  The stats do not become Mew stats.  The attacks aren't Mew attacks.
Although this glitch is most known for being able to change a Magikarp into a Mew, it is not limited to that.  Any Pokémon can be changed into Mew in this way, as long as a Pokémon who is either a pure Water-type or has a Type 2 of Water is the first Pokémon in the party.
