The first one is the Pokémon's number.
The second is the Pokémon's name.
The third is for the type of its Poké Assist.
The fourth is the field move's name.
The fifth one is for the Field Move's number
The sixth is for the number of loops needed for capture
Add yes in the seventh to be able to add notes.
The eighth is for optional notes.

Example:
