That's basically just ripped direct from the ROM, and not prettied up too much. The Pokemon are sorted in order of percentage chance they appear: 25% 15% 15% 10% 10% 10% 5% 5% 4% 1%. So for instance, Pikachu appears in the Viridian Forest rarely, at level three 4% of the time, and level five 1% of the time. You'll notice that there's not too much difference between this and Red and Green, aside from some small changes in the Safari Zone and Seafoam Islands and such. For the most part, in the places that Red and Green differ, Blue will go with one or the other, and stay exactly the same otherwise. Mostly.
Surf and Fish data are identical, just like Red and Green. The number that shows after the zone name is an indicator of how often Pokemon will show up. So, it'll be high in places like routes with tall grass, but low in caves and such because you don't encounter them as often. Even if it sometimes doesn't seem that way. ;)
