Known as "a quiet mountain retreat", Blackthorn's main attraction is the Blackthorn Gym, the last of Johto's official Gyms.  Due to its isolation (the only way into town is through Ice Path), there isn't much else of interest here.
Blackthorn City is known as a quiet mountain retreat, and lies isolated in the northeast part of the region.  The main attraction is the Blackthorn Gym, the eighth and final of the Johto Gyms.  To the north is Dragon's Den, a training ground for Dragon specialists.  On Saturdays, Santos of the Week Siblings can be found southeast of the Gym.

In-game trade
{| class="expandable" align="center" style="background: #crystal color; roundy; border: 5px solid #crystal color light;"
The girl in the first house you pass by is looking to trade. She is willing to trade her Dodrio for your Dragonairfemale. While Dodrio are rare, a Dragonair (and then Dragonite) will end up stronger in the long run.

Move Deleter
To the left of the Poké Mart is the Move Deleter's house.  This guy can help Pokémon forget unwanted moves (like HM moves, or moves that can't be traded to the Generation I games).

Later...
A New Trade
Visit Blackthorn on a Saturday, and jump down the ledges to the east of the Gym.  Santos will be standing here, waiting to give you a Type-enhancing item, which strengthens Ghost attacks.
Inside the eastern house is a girl looking to trade.  She offers her Dodrio in exchange for your female Dragonair.  The choice is yours, but a Dragonair's evolved form, Dragonite, will end up stronger in the long run.

Blackthorn Gym
Move Deleter
The Move Deleter resides in the house near the Poké Mart, and can help a Pokémon forget any of its moves.  This is especially helpful for deleting HM moves, or moves that are exclusive to Generation II and cannot be traded through the Time Capsule.

The Blackthorn Gym is devoted to the Dragon, so your recent acquisitions from Ice Path will do well here. You will also need to bring a Pokémon that knows Strength with you to form a bridge to the Gym Leader.
The Blackthorn Gym is devoted to Dragon Pokémon.  Dragons take less damage from Grass, Fire, Water, and Electric moves, and are only vulnerable to Dragon- and Ice attacks.  Almost any Pokémon that you caught in Ice Path will do well here. Reaching the Gym Leader requires dropping boulders from the floor above to form a path, so bring along a Pokémon that knows Strength.

Battle past Cooltrainer Paul, and take the stairs to the second floor.

North of the stairs, you'll see two boulders side-by-side.  Neither one of these will help you get to the Gym Leader, so head south.  Beat Cooltrainer Fran, and you'll see two boulders in the southeast area.  Push the right one down to the south wall, and shove the left one ↑9, →2 to drop it down the holeContinue north, defeat Cooltrainer Cody, shove the next boulder to the east wall, and push the last south, dropping it down the nearest hole.  Take the eastern stairs down to the center of the Gym's main floor.
Take the stairs to the second floor and go south.  Walk past Cooltrainer Fran to find two boulders in the southeast corner; move the right one to the south wall, and the left one up to reach the break in the floorGo to the northeast corner and drop the last boulder through the hole.  Take the eastern stairs down to the center island and beat the last two Cooltrainers to reach Clair.

Battle past Cooltrainers Mike and Lola, and you'll reach Clair at the back of the Gym.
Each of Clair's Dragonair can paralyze an opponent with Thunder Wave, while her Kingdra can do the same with DragonBreath.  Use status ailments to your advantage, and replenish your health often so you are not caught off-guard by Kingdra's Hyper Beam.

Clair can't accept that she lost the battle.  She tells you that you're still not ready for the Pokémon League, and that you should take the "dragon user challenge".  She mentions the Dragon's Den, located across the pond behind the Gym, telling you to bring her the Type-enhancing item from there to be accepted as a true dragon user.  Only then will she give up your last badge.  Grab a Pokémon with Surf and Whirlpool, and head inside the cave.

Clair refuses to accept defeat.  She claims that you're still not ready for the Pokémon League, and tells you to take the dragon user challenge.  She informs you of the Dragon's Den, a large cave located under the city, and tells you to retrieve the Type-enhancing item from there to be accepted as a true dragon user.  Only then will she give up your last badge.  Bring along a Pokémon with Surf and Whirlpool, and cross the lake behind the Gym.

Enter the cave, and take the stairs down to the main cavern.  From the entrance, surf to the southwest, and you'll be stopped by a whirlpool.  Once you get past it, head around to the east behind the shrine, and you'll find the Type-enhancing item on the eastern shore.  Clair doesn't show up yet, so step inside the shrine to meet the clan's elder.  He gives you a test; 5 questions to determine your attitude towards Pokémon.  Avoid the obvious negative answers ("Underling", "Cheating", etc) to pass the elder's test.  Leave and re-enter the cave (with no more than 5 Pokémon), and the elder will reward you with a Dratini; the young dragon's species is rare enough, but this individual knows the ExtremeSpeed move, which they don't normally learn.

Clair enters the shrine, fully expecting you to fail the elder's test. She is furious that you've passed the test while she has not. The elder steps up, demanding that she award you the Rising, which she reluctantly does. The final Johto badge allows the use of Waterfall outside of battle, and commands attention from all Pokémon, regardless of level. Clair leaves, but stops you on your way out to give you 24 as an apology.

As soon as you leave the cave, Elm calls you. He asks you to visit him at the Lab, and since you have to travel through New Bark anyway, it won't be a problem. Heal up your team, and head south to 45.

45 is a rugged route leading down from the mountains. The path splits and recombines multiple times, and many small cliffs prevent northbound travel.  The east entrance to Dark Cave is just outside of the city, and the route eventually turns west near a small lake.

Dark Cave
While it was accessible once you had reached 31, you didn't have the moves (Flash, Rock Smash, Surf, Strength) needed to fully explore this shadowy cave. This cave leads to 31 and 46
Blackthorn Side
From the northeast entrance near Blackthorn, walk south to the water's edge.  Surf directly south, and climb onto the nearby rock ledge to find a Revive.  Follow the path to the west, and you'll find 13 near a ledge.  Backtrack to the lake, and swim across to reach the northern shoreFollow the trail north, then turn towards the southwest.  Turn to the north, and speak to the blue-haired guy to get a pair of Type-enhancing item, which enhance Dark attacks.  Head south walk through the doorway to reach the cave's western side.

Turn right and surf south across the lake.  Grab the Revive on the southeast platform, and go west along the water's edge to find 13.  Surf north and go west along the platform.  Make a U-turn to reach the northwest corner; the man here will give you the Type-enhancing item, an item that enhances Dark attacks.  Head south and jump the two ledges to reach the opening to the Violet side.
Violet side
Surf to the west, and jump the ledge to find a Potion if you didn't already.  Go south and use Rock Smash to follow the tunnel east.  Smash another rock and loop around the northeast corner to find a Potion.  Jump the ledge and head south to find a Status ailment healing item and a Battle item.  Continue south to reach the 46 exit.

46 holds the south entrance to Dark Cave, sitting atop the small cliffs.  A grove of berry trees stands off to the west. Go south to the gate, and cut through 29 again to return home to New Bark.

46 is a small mountainous path that continues from where Route 45 end. Make your way south, and you'll eventually reach the gate that connects to 29. From there you can head to the east toward New Bark to see Elm.
